Describe the bug

I cannot include any devices (Two Zooz Zen77 available for testing) I have attempted countless variations of: excluding the device, including the device, factory reset, factory reset + exclude, factory reset the stick... I have tried traditional include, SmartStart include, S0 include.

Every attempt fails with these messages:
2024-11-25T06:03:16.206Z CNTRLR handling add node request (status = Failed)
2024-11-25T06:03:16.207Z CNTRLR Adding the node failed
2024-11-25T06:03:16.208Z CNTRLR stopping inclusion process...

To Reproduce

Attempt to include a device in Z-wave JS UI

Expected behavior

Device would be included, interviewed and functional

Additional context

The hardware is:
Zooz ZST39, FW: v1.50, SDK: v7.22.1 (Upgraded from 1.40 before any testing due to notice in Z-Wave JS UI)
Zooz Zen77, FW appears to be 4.40. (2x total, only one installed. Same errors with either)
ZST39 was using an USB2 port with an extension for all tests in the logs

This comment on another issue is one of the few references to these errors I could find: zwave-js/zwave-js-ui#1069 (comment)
"status = Failed is the controller telling us that it cannot include devices right now.
If that doesn't change, your best bet is: shut down the host, pull the stick put it back in and start again."

I have tried that a few times of course, sadly no improvement.

Odd thing is, by using the Z-Wave PC Controller in Simplicity Studio, I am able to pair the Zen77 dimmer - but only if I preprovision it in Smart Start. No other provisioning method works. While using this tool, I can also control the dimmer with no issues. Moving the stick back to Z-Wave JS UI results in an unkown entry in the list of nodes.

I have attached drivers logs that contain dozens of attempts of all types as mentioned above.

I'm new to Z-Wave, hopefully all the necessary information is here, please let me know if any further information is required.
